Dissecting Aneurysms on Vertebral Artery and its Branches.

Abstract
BACKGROUND
Dissecting aneurysms of the vertebral artery are rare. Diagnosis and treatment of such condition aneurysm has a potentially higher degree of technical difficulty. The authors analyzed the clinical features and treatment modality for dissecting aneurysms arising from vertebral artery and its branches. METHODS: At the authors'institution between April. 2001 and Sep. 2004, 18 patients were diagnosed and treated for dissecting aneurysms of vertebral artery and its branches. The medical records and neuroimaging studies of the patients were reviewed retrospectively. RESULTS: 18 patients were comprised of 8 female and 10 male patients aged from 24~69 year old (mean: 44.2). Of the 18 patients, 8 patients (44.4%) had subarachnoid hemorrhage. 6 patients with subarachnoid hemorrahge were in good neurological status before treatment. Four patients were treated with transcranial surgery and 7 patients with endovascular treatment. the other 7 patient were merely treated with conservative care. All patients were diagnosed with digital subtraction angiography (DSA), but Magnetic resonance angiography (MRA) showed 12 dissecting aneurysms, 2 suspicious cases and 3 normal finding. In the case of anterior inferior cerebellar artery (AICA) dissecting aneurysm, MR angiography showed no visible AICA. Posterior inferior cerebellar artery (PICA) was occluded in postoperative DSA after wrapping of PICA for dissecting aneurysm of proximal protion of AICA. But the patient show no neurologic deficit. One patient developed bilateral vertebral artery dissecting aneurysms. One patient treated only conservative care revealed normal DSA finding on follow-up study. CONCLUSION: DSA is the only accurate diagnostic tool for vertebral dissecting aneurysm. Proximal GDC occlusion of vertebral artery may be a safe and effective therapy for patients with vertebral artery dissecting aneurysm. For proper decision of safe treatment modality, one should consider vertebral artery dominancy, collateral circulation, PICA invasion.

서     론


  
두 개강내 추골동맥 및 그 분지부의 동맥류(aneurysm of intradural vertebral artery and its branches)는 두개강내에서 발생하는 뇌동맥류 중 약 3% 미만을 차지하지만 박리성 동맥류의 발생율이 다른 부위에 비해 높아서 약 28% 정도로 알려져있다.3)9)11)20) 또한 후두와에 위치하여 뇌간과 뇌신경이 추골동맥과 근접하게 붙어있어 수술적 접근이 상당히 어렵고, 또한 수술적 접근중 동맥류 주위에 위치한 뇌신경들의 손상시 술후 변성, 연하장애나 이로 인한 흡인성 폐렴 등이 초래될 있는 치료가 어려운 동맥류이다.4)5)10)11) 최근 들어 뇌혈관 촬영기법의 발달로 인한 진단의 증가와 뇌혈관내 수술법의 발전 및 혈관내치료기법의 발달로 그동안 치료가 불가능했던 박리성 뇌동맥류의 치료가 가능해지고 있다.1)20) 이에 척추동맥과 그 분지의 박리성 뇌동맥류에 대한 진단으로 자기공명 혈관촬영술과 디지탈 감산 혈관촬영술과 차이를 비교해보았으며 임상 분석 및 치료방법에 대해 알아보고자 한다.

대상 및 방법

   2001년 4월부터 2004년 9월까지 척추동맥과 그 분지에서 발생한 박리성 뇌동맥류 18례를 대상으로 임상 및 방사선학적인 소견, 치료방법을 알아보았으며 진단적 가치로서의 디지털 감산혈관조영술과 자기공명혈관조영술을 비교해 보았다. 치료는 개두술 4예, 혈관내 치료 7예, 나머지 7예에서는 보존적 치료를 시행하였다.
   임상적인 양상은 신경학적 검사을 시행하여 판정하였고 결과는 퇴원당시의 Gloscow Outcome Scale (GOS)을 기준으로 추적 관찰하였다. 모든 의무기록과 방사선 검사 결과를 분석하였다.

결     과

   대상 환자는 남자 10례, 여자 8례였으며 연령분포는 24세에서 69세로 평균 44.2세였다(Table 1). 거미막하출혈이 8례였고 9례는 허혈성 증상을 보였고 2예는 두통을 보였고 뇌경색 중 3예는 외측연수증후군(lateral medullary syndrome)을 보였다(Table 1). 위치는 우측이 6례, 좌측이 11례였으며 1례는 양측으로 발생하였다(Table 1). Mizutani 분류법에 의하면 type I이 4례, II가 9례, III가 3례였으며 후하소뇌동맥 자체와 전하소뇌동맥 자체에 생긴 예가 각각 1예씩 있었다(Table 1, 2). 18례 모두 디지털 감산 혈관조영술에서 박리성 동맥류로 진단되었가. 그렇지만 자기공명 혈관촬영술에서는 12례가 확진되었고 2례는 박리가 의심되는 정도였으며 3례는 정상으로 보였다(Table 1). 전하소뇌동맥의 박리성동맥류의 경우 자기공명혈관조영술에서 전하소뇌동맥자체가 보이지 않았다(Table 1). 양측성으로 발생한 1예에서는 보존적 치료 후 추적 조사 중이다. 보존적 치료를 하였던 7예 중 뇌지주막하출혈을 보여 처음 상태가 나빴던 2예는 재출혈로 사망하였으며 그 외 허혈성 증상을 보였던 5예 중 1예는 뇌경색이 반복해서 생겼고 나머지 4예는 증상이 호전되었으며 그중 1예에서는 디지털감산 혈관조영술에서 자연치유되는 소견을 보였다.
   수술 적 치료는 뇌지주막하출혈이 있었고 혈관의 사행성 경로로 인해 혈관선택이 불가능했던 2예는 근위부 결찰술을 하였다(Table 1). 그 외 6례는 Guglielmi Detachable Coil(GDC) 색전술을 하였으며 가벼운 뇌경색을 보이고 혈관조영술상에서 단순하게 좁아져 보이던 4례는 보존적 치료 후 추적조사 중이다(Table 1).
   전하소뇌동맥에서 생긴 1예는 rostral trunk에서 생긴 것으로 suboccipital approach로 caudal trunk는 보존하면서 trapping을 시행하였고 수술 후 아무런 신경학적 이상소견 없이 퇴원하였다(Fig. 1). 후하소뇌동맥에서 생긴 1예는 최외측접근법(far lateral approach)으로 성공적으로 wrapping하였고 수술후 혈관조영술에서 후하소뇌동맥이 보이지 않았으나 환자는 이상 소견 없었다(Fig. 2). 뇌간부와 소뇌 및 후두엽의 뇌경색이 있었던 1예는 좌측 척추동맥 박리성 뇌동맥류를 GDC로 치료후 약 3개월 후 추적 조사한 디지털감산 혈관조영술에서 우측 척추동맥이 약 80% 막혀 stent 삽입술을 시행하였다(Fig. 3).

고     찰

   후순환계의 뇌동맥류는 전체 두개강 내 동맥류의 약 15%를 차지하고, 이중 추골동맥-후하소뇌동맥 분지부의 동맥류는 약 20%를 차지하는 것으로 알려져 있다.2)4) 추골동맥 동맥류를 위치별로 구분을 하면 추골-기저동맥 분지부, 추골동맥, 추골-후하소뇌동맥 분지부의 동맥류로 나눌 수 있고, 특성별로 구분을 하면 낭상, 박리성, 방추상, 거대동맥류 등으로 구분을 할 수 있다.11)20) 하지만 종종 동맥류의 특성이뇌혈관 조영영상뿐 아니라 수술 시야에서 조차 구분하기 힘든 경우도 많다.11)
   추골동맥 동맥류의 증상으로는 뇌지주막하 출혈로 인한 증상 발현이 제일 흔하고, 동맥류가 커져 뇌간을 압박하여 증상이 발생하거나 뇌신경 압박이나 허혈로 인한 증상이 있을 수도 있다.6)11)31) 또한 동맥류가 뇌간 근처에 위치하기 때문에 동맥류의 파열과 함께 갑작스런 의식 소실, 호흡정지 등이 발생하기도 한다.20) 본 연구에서는 상대적으로 출혈보다 뇌경색으로 인한 증상을 호소한 예가 많았는데 두통, 어지럼증 등 비특이성 증상을 호소하고 뇌전산화 단층 촬영이나 MRI상 뇌경색을 보이는 환자에서 혈관조영술을 하는 경우가 많아져서 박리성 뇌동맥류가 더 많이 진단되는 것이라 생각되었다.
   두개강 내에서 발생하는 비 외상성 거미막하 출혈의 3
~7%는 박리성 뇌동맥류에서 발생하는 것으로 보고되고 있다.16)17)24) 거미막하출혈로 사망한 부검례의 4.7~7.7%와 두개강 내의 모든 뇌동맥류의 3.2%에서 척추동맥의 박리성 뇌동맥류가 발견된다.24)30) 두개강 내에서 박리성 뇌동맥류 발생의 가장 흔한 부위는 중뇌동맥과 척추동맥이다. 척추동맥의 경우 5~10%에서 양측성으로 발견된다.30) 따라서 한쪽에서 발생하였을 경우 추후 반대쪽에서 발생할 가능성이 있기 때문에 치료시 박리된 혈관을 보존할 수 있는 방법을 먼저 고려해야 한다.
   동맥의 박리(dissection)는 대부분 건강한 환자에서 특별한 원인 없이 자발적으로 발생하지만, 매독성 동맥염, pol-yarteritisnodosa, fibromuscular dysplasia, Marfan 증후군, mucoid degeneration of arterial media 등의 일차적인 동맥병변(primary arteriopathy)이 있는 경우 동반되기도 하며, 가벼운 두부외상이나 수술 중 동맥의 조작등과 관련되어 나타나기도 한다.32) 조직학적으로 두개강내 동맥의 중막(media)과 외막(adventitia)은 두개강외의 동맥보다 얇다. 즉 중막의 external elastic membrane이 약하고 중막내의 elastic fiber의 양이적다.25) 따라서 동맥의 내막과 중막사이에 박리면(dissection plane)이 생기면 이는 곧 약한 중막과 외막으로 파급되어 거미막하 출혈을 유발한다. 그러나 두개강 외의 동맥에서 발생한 박리성 동맥류의 경우, 박리면은 내막과 중막 사이에 국한되어 나타나고 이 부위에 혈종이 생겨서 혈관이 좁아지거나 혈전의 형성원이 되어 환자는 주로 뇌허혈 증상을 나타내게 된다. 또한 두개강 내의 뇌혈관은 자체의 vasa vasorum의 발달도 빈약하기 때문에 박리면 발생 후 정상적인 혈관의 치유과정(healing process)이 제대로 이루어지지 않는 것도 거미막하 출혈이 많은 이유 중 하나이다.3)
   진단은 극심한 두통을 호소하는 환자에서는 Brain CT를 이용하여 거미막하 출혈을 확인후에 혈관조영술로 진단할 수 있으며 비 특이적인 증상을 보이는 경우는 MRI를 이용한다. MRI에서 박리성 뇌동맥류는 박리면이 발생한 혈관 주위에 eccentric high signal collar의 소견이 보인다. 혈관조영술상 혈관이 부분적으로 확장되는 소견인 pearl sign이 선행되고 혈관의 내경이 부분적으로 좁아지는 소견인 string sign이 전형적으로 나타나며 그 외에도 염주알 모양(beaded appearance), double lumen, gradual tapering, 또는 완전폐색 등의 다양한 소견을 보인다.7)31) 전형적인 pearl & string sign은 낭성 동맥류와 주변혈관의 연축현상으로 오인되기도 한다. 그러나 본 연구에서 3예는 DSA에서 박리성 뇌동맥류로 진단이 되었지만 MRA (Magnetic resonance angiography)에서는 정상소견으로 나왔다. 아직 MRA 단독으로는 진단적 가치를 가지기 어려우므로 후순환계의 박리성 뇌동맥류가 의심되면 반드시 DSA를 시행하여야 할 것으로 생각되었다.
   두 개강내 박리성 뇌동맥류의 수술적 치료는 clip을 이용한 박리면 근위부 동맥의 결찰, trapping, 또는 wrapping 등과 혈관재건술(revascularization)이 있다. 척추동맥의 박리성 뇌동맥류의 경우 병변이 비우성척추동맥에서 발생하였거나 양측의 동맥의 크기가 비슷하면 clipping이나 trapping 등의 방법을 선택할 수 있고, 병변이 우성척추동맥에서 발생한 경우는 wrapping이나 혈관재건술을 선택할 수 있다. 혈관내 수술법을 이용한 치료로는 색전술을 시도할 수 있으며 이 경우 혈관의영구적인 폐색을 유도하기 전에 시험적 폐색술(test occlusion)을 시도하여 신경학적 결손의 발생이나 진행유무를 확인하여야 한다. 색전술시 사용하는 색전물질로는 balloon과 coil이 있으며 이를 박리면의 바로 근위부(just proximal)까지 이동시킨후 박리성 뇌동맥류를 폐색시키고 이곳으로 유입되는 혈류를 차단한다. 뇌혈관내 색전술의 장점은 개두술이 필요없으며 영구적인혈관폐색을 유도하기 전에 시험적 폐색술을 시도하여 신경학적인 변화를 미리 평가할 수 있다는 것이다. 그러나 색전술 시행도중에 재출혈의 위험성이 있으며, 후순환계(posterior circulation)에는 풍부한 측부순환(collateral circulation)이 발달되어 있으므로 불완전한 색전술을 방지하기 위하여 balloon이나 coil 등의 색전물질을 박리면이나 박리면의 바로 근위부에 위치시켜야 하며, post-PICA type의 박리성 뇌동맥류의 경우 post-PICA의 VA부위에는 뇌간으로 가는 perforator혈관이 많기 때문에 폐색시킬 동맥분절의 크기를 최대한 짧게해야 하는 어려움이 있다.5) Balloon이나 coil에 의해 막힌 혈관의 원위부에는 점진적인 혈전이 형성되며 이 부위에서 기시하는 perforator나 PICA로는 반대측 척추동맥이나 기저동맥으로부터 역행성으로 혈류가 형성되는 경우도 있다.
   척추동맥의 박리성 뇌동맥류를 수술적 또는 뇌혈관내 색전술을 이용하여 치료 할 때에는 환자의 술전 신경학적 상태, 나이, 다른 전신질환의 유무 등과 함께 척추동맥의 dominancy, PICA와 동맥류와의 관계, 양측성 병변의 유무, 두개강 내 다른 부위에 동반된 동맥류나 동정맥 기형의 유무 등을 고려해야 하며 반드시 시험적 폐색술을 시행하여 신경학적 변화 등을 확인하여야 한다. 최근에는 박리성 뇌동맥류가 발생한 혈관의 개통성(patency)을 유지하면서 박리면을 폐색시킬 수 있는 stent & coil 기술이 개발되어 박리성동맥류의 치료에 가장 먼저고려해야할 치료방법이 되고 있다.9)13) 드물게 박리성 뇌동맥류가 저절로 치유되는 사례가 보고되고 있는데12)14)21)23) 뇌출혈의 증상이 없고 혈관조영술상에서 경도의 협착만을 보인다면 혈관조영술을 추적검사하면서 주의깊게 관찰하는 것도 한 방법이라 생각된다.
   후하소뇌동맥의 낭성 뇌동맥류는 드문 것으로 보고 되고 있으며 후하소뇌동맥의 주행에 따라 anterior medullary segment, lateral medullary segment, tonsillo-medullary segment, televelotonsillar segment, cortical segment로 나누었을때18)29) 주로 medullary segment와 televelotonsillar segment에서 호발하는 것으로 알려져 있다.19) 박리성 뇌동맥류는 아주 드문 것으로 보고 되고 있으며8)15)18)22)26)27)28) 본 연구에서는 그 증례가 anterior medullary segment에서 생겨 lateral medullary setment로 확장된 경우였으며 치료는 개두술 후 뇌동맥류 부분을 wrapping하였는데 3개월후 시행한 DSA상 후하소뇌동맥이 막혔으며 전하소뇌동맥에서 측부순환이 발달되는 소견을 보였다. 환자는 수술 전 두통과 경부강직 외 다른 증상은 없었으며 수술 후 별다른 신경학적 손상 없이 지내고 있다.

결     론

   척추동맥과 그 분지에 발생하는 박리성 뇌동맥류는 점차 진단의 빈도가 증가하고 있다. 아직 자기공명혈관조영술은 그 진단의 정확도가 디지탈 감산 혈관조영술에 미치지 않으므로 정확한 진단을 위해서는 디지탈 감산 혈관조영술을 시행해야 한다. 혈관내 치료법은 비교적 안전하고 비침습적인 방법이라고 생각된다. 안전한 치료법을 결정하기 위해 척추동맥의 dominancy, 측부순환의 유무, 중요분지의 침범유무 등을 잘 파악하여야 한다.
