Complications of Cerebral Revascularization Surgery.

Abstract
A variety of procedures have developed for the surgical augmentation of collateral circulation to the brain in some cerebral or cerebellar lesions such as hemodynamic ischemic stroke, giant aneurysm of circle of willis or skull base tumor. Carotid endarterectomy and extracranial-intracranial arterial bypass (EIAB) are included in surgical revascularization (SR), and thrombolysis and transluminal percutaneous angioplasty and/or stenting (PTAS) are included in endovascular revascularization (ER). This article focuses the revascularization procedure related complications in patients treated with surgical or endovascular revascularization.

서     론


   폐쇄성 뇌혈관질환, 거대동맥류 혹은 뇌종양 등의 여러 가지 뇌질환에 있어서 뇌를 뇌허혈이나 뇌경색으로부터 보호하기 위하여 시행하게되는 직접적인 cerebral revascularization에는 크게 보아 surgical revascularization과 endovascular revascularization으로 나눌 수 있다.
   SR에는 가장 많이 활용되고 있는 superficial temporal artery-middle cerebral artery(STA-MCA) anastomosis 혹은 occipital artery-posterior cerebral artery(OA-PCA) anastomisis와 같은 low-flow bypass와 saphenous vein이나 radial artery를 이용하는 high-flow bypass 등이 있다.
   최근에 많이 활용되고 있는 ER에는 급성기의 폐쇄성 뇌혈관질환에서 혈전용해제를 이용한 thrombolysis나 협착성 뇌혈관질환에 이용되는 angioplasty와 stenting 등이 있다.
   이상과 같은 SR이나 ER의 시술로 인하여 나타날 수 있는 합병증에 대하여 기술하겠다.

1. Surgical revascularization

1) 천측두동맥-중대뇌동맥 문합술(STA-MCA anastomosis)
  
1967년 Donaghy와 Yasargil10)에 의해 처음 시술되었으나, 1985년에 이 수술은 폐쇄성 뇌혈관질환의 예방에 도움이 되지 못한다25)는 발표 이후로 이 수술의 활용은 급격히 감소되었다.
   그러나 최근에 와서 내과적 약물치료로 효과가 없는 hemodynamic cerbral ischemia를 가진 환자이거나, 뇌종양, 거대뇌동맥류에서 주동맥의 근위부를 치료 목적으로 폐쇄시킬 필요가 있거나, moyamoya병 등에서는 STA-MCA 문합술과 같은 측부순환을 만들어주는 수술이 뇌에 혈류증가를 위하여 유용하게 활용되고 있다.
   기술적인 면에서 이와 같은 low-flow SR의 수술에 있어서는 수술이 시행되는 부위자체가 두피와 뇌표면이기 때문에 별로 위험이 따르는 수술이 아니므로 특별한 합병증이 없는 경우가 많다.
   Sundt 등23)이 403명의 환자에서 시행한 415례의 STA-MCA 문합 수술에서 수술후 30일 이내에 발생한 사망자는 5명으로서 1%이었으나 그중 3명의 사망원인은 심장질환이었다. 수술 후에 새로 발생한 크고 작은 신경학적 결손까지 모두 합치면 합병증이 12%이었으나 일과성 허혈 발작(transient ischemic attack, TIA)만 있었던 환자에서는 3%에서만 합병증이 나타났고, major stroke으로 인한 morbidity는 수술 전에 신경학적으로 불안정한 환자에서 주로 발생하였다.
  
이 수술과 관련되어 나타난 가장 흔한 합병증은 뇌경막하수종으로서 이는 주로 위축된 뇌표면위의 지주막에 수술로 인하여 생긴 틈 때문에 발생하는 것이지만, 대부분에서 자연적으로 흡수되므로 큰 문제가 되지 않으나, 뇌경막하수종외 에도 뇌경막하혈종이 가끔 합병될 수 있다.
  
다음으로 TIA나 가역성 허혈성 신경학적 결손(reversible ischemic neurological deficit, RIND)이 나타날 수 있는데, 이는 STA-MCA와 같은 수술은 뇌관류가 경계부위에 있는 환자들에서 자주 시행됨으로써 혈압이나 심장 박출량의 변화나 혹은 뇌내 steal 현상으로도 뇌허혈 증상이 발생할 수 있다. 대부분에서 bypass graft가 어느 정도 확장되어 뇌허혈을 보호할 정도의 뇌혈류량이 공급되기까지는 최소한 5일 내지 6일이 경과되어야 하기 때문에, 수술 직후에 뇌허혈 증상의 예방을 위하여 정상적인 혈류량이나 혈압을 유지시키는 것이 매우 중요하다고 본다.
  
뇌내출혈도 Sundt 등23)에 의하면 415건의 수술에서 2명에서 발생 되었는 바, 이는 둘 다 수술 부위와는 연관성이 없는 hyperfusion의 결과에 의한 것으로 설명하고 있다.
  
Bypass 수술후에 뇌동맥의 협착부위가 완전폐색으로 진행되는 경우가 보고되고 있다. Sundt 등23)에 의하면 131건의 STA-MCA 문합술후 2례에서 내경동맥의 siphon 부위의 협착이 완전폐색으로 진행되었다.
  
두피괴사 혹은 감염도 발생할 수 있으나, 특히 두정지와 전두지를 모두 활용했을 경우에 흔히 발생한다.

2) 복재정맥(Saphanous vein) bypass graft
  
High-flow bypass를 위하여 가장 많이 활용되고 있는 saphenous vein을 이용한 graft에서의 합병증으로는 early graft thrombosis, subacute graft occlusion과 late occlusion으로 나눌 수 있다.22)

(1) Early graft thrombosis
   수술후 첫수일 혹은 2~3주 내에 일어나는 graft occlusion은 대부분에서 혈전증(thrombosis)에 의하는 바, 그 원인으로는 외상에 의한 혈관내막박리(intimal desquamation), slow graft flow, 혈액의 응고성(coagulability)들이다.

(2) Subacute graft occlusion
   Graft 후 1~2년이 지나면서 나타날 수 있고, 폐색의 원인으로서는 혈관내막증식(intimal hyperplasia)과 중막섬유조직증식(medial fibrosis), valve fibrosis, 동맥류성확장(aneurysmal dilatation), clamp stenosis와 suture stenosis 등이다.

(3) Late occlusion 
   대부분에서 죽상경화증(atherosclerosis)가 주원인으로 알려져 있다.
   Femoropopliteal graft occlusion율은 수술 후 첫 1~3년 동안에 20%4) -37.4%,9) 5년동안 폐색율은 32~42%,4)8)9)24) 10년 폐색율은 55%로 보고되고 있다.
   다른 보고들에 의하면 급성폐색율은 10%, 1년 폐색율은 10~35%2)3)12)13)18)21)이며 그 후론 매년 2~3%율로 증가한다고 한다. 어떤 부위의 vein bypass graft이든 가장 중요한 것은 vein harvesting와 preparation에서의 기술적인 측면이란 점이 지적되고 있다. 두개강내 폐쇄성 뇌혈관질환이나 거대동맥류에서 Sundt등이 시행한 132예의 sapheuous vein bypass graft등에서 graft patency는 처음 65 례에선 74%이었으나, 다음에 시행한 67례에선 94%로 향상되었다. 후방순환에서의 수술결과가 좋았던 증례는 폐쇄성혈관질환 환자 77명중 55명인 71%, 거대동맥류 환자 9 명중 4명인 44%에서 나타났고, 전방순환에서는 폐쇄성혈관질환인 경우 26명중 15명인 58%, 거대동맥류 환자 20명중 16명인 80%에서 좋은 결과를 얻었다. 평균 graft blood flow도 100~150 ml/minute로 유지되었다.
   Regli등20)이 시술한 extracranial-intracranial saphenous vein bypass graft(EISVG) 202증례(폐쇄성뇌혈관질환 127례, 거대동맥류 74례, 뇌종양 1례)등에서 수술 후 1년동안에 있었던 graft 실패율은 76%이며 모든 graft 실패율 중의 42%는 처음 24시간 내에 일어난다고 하였다.
   수술후 1년, 5년과 13년에 각각의 cumulative patency는 86%, 82%, 73%로서 EISVG의 장기적인 개통율은 우수하다고 보았으며, 수술 1년 후부터 매년 실패율을 1~1.5%로 보았다. 결론적으로 대부분의 정맥폐색은 수술 후 초기에 기술적인 문제로 인하여 발생하므로 정맥의 준비, 위치, 문합시의 술기에 최대한의 주의가 필요하다고 본다.

3) 요골동맥(Radial artery) graft bypass
  
내경동맥의 해면정맥동 혹은 paraclinoid 부에서 발생한 거대동맥류 43례에서 내경동맥을 경추부에서 폐색하고 요골동맥이식(radial artery graft)으로 내경동맥을 연결한 43명의 환자 중 20명에서 평균 5년 이상의 장기추적에서 단 1례에서도 동맥의 폐색은 없었으나, 2례에서 graft의 협착이 나타났다.15) 수술 수기는 주로 외경동맥과 M2 사이에 요골동맥을 위치시킨 직후 내경동맥의 근위단을 폐색시켰다. 이 방법은 기술적인 면에서나 혈관문합술 동안의 긴 폐색시간을 요하는 복재정맥 bypass graft에 비하여 훨씬 안전하고 쉬운 시술로 알려져 있다. 그러나 요골동맥은 vaso vasorum을 통하여 영양을 공급받는 muscular artery이기 때문에 동맥벽에 변성변화가 와서 동맥의 급성 및 만성폐색이 올 수 있다는 점을 항상 염두에 둘 필요가 있다.

2. Endovascular revascularization

1) 혈전용해(Thrombolysis)
  
혈전용해는 동맥내 혹은 정맥내에 혈전용해제의 주입으로 가능하며, 단독으로 혹은 PTAS 직전에 활용되고 있다.
   미국의 NINDS(National Institute of Neurological Disorders and Stroke)에서 시행한 임상연구를 근거를 하여 AHA(American Heart Association)에서는 급성 허혈성뇌졸중 환자에서 증상 시작 3시간 이내에 recombinant tissue plasmonogen actirator(t-PA)를 정맥(0.9 mg/kg의 용량으로 최대용량 90 mg까지) 내로 주입할 것을 권장하고 있다.1)19)
   반면에 동맥내 혈전용해술은 동맥폐색 부위에 높은 농도의 혈전용해제를 주입시킬 수 있고 angioplasty나 stenting을 동반시술하게 할 수 있는 장점이 있을 뿐 아니라 뇌졸중 증상 시작후 6시간까지는 주입해도 안전한 것으로 보인다.5)
   del Zoppo등5)이 시행한 PROACT(Prolyse in Acute Cerebral Thromboembolism) 연구에서 중대뇌동맥이 색전으로 폐쇄된 26명의 환자에서 6시간 이내에 prourokinase를 주입한 후 14명의 중대뇌동맥이 폐쇄된 대조군과 비교하여 동맥개통율에서 58% 대 14%로 치료군이 좋았고, 증상있는 뇌내출혈에서 15% 대 7%로 치료군에서 많았지만 치료후 90일에서의 임상결과는 치료군에서 더 좋은 경향을 보여주었다.
   이와 같은 혈전용해에서 나타날 수 있는 합병증으로는 출혈성 합병증과 비출혈성 합병증으로 나눌 수 있다.
  
출혈성합병증은 혈전용해에서 초래될 수 있는 가장 심각한 합병증으로써 t-PA로 치료한 뇌졸중환자 104명 중에서 뇌내혈종이 많게는 11%,6) 출혈성 뇌경색으로 전환된 경우가 많은 경우에 51명 중에서 47.1%26)까지 보고되고 있으나, 출혈성 뇌경색에서는 신경학적으로 나빠지는 경우가 드문편이나 뇌내혈종에서는 반수 이상에서 결과가 나쁘다.
   Urokinase나 streptokinase를 사용한 동맥내혈전용해에서는 del Zoppo 등7)의 환자 20명중 4명(2%)에서 뇌내혈종을 보고하고 있고, Hacke 등14)은 환자 43명중 4명에서 뇌내혈종을 보고하고 있다.
  
비출혈성합병증에는 색전형성과 재관류손상으로 나눌 수 있는데, Liebeskind 등이 색전형성에 의하여 임상적으로 나빠진 두 환자17)를 보고하였고, Kondstal 등16)은 뇌졸중에서 t-PA 치료후 뇌부종으로 인한 사망한 두 환자를 보고하였다.

2) 경피적혈관성형술 및 스텐트삽입술(PTAS)
  
Cerebral ER에 많이 활용되는 PTAS는 첫째로 협착된 혈관의 단면을 넓힘으로서 혈류량을 증가시키고, 둘째로는 혈류동학적 개선을 통하여 혈전형성을 감소시키고 나아가서 죽종판의 진행과 물리적성질을 변화시키고자 하는데 있다.
   내경동맥내막절제술(carotid endoarterectomy)을 시행하기에 위험도가 높을것이 예상되는 반대측 내경동맥 폐색(contralateral internal catotid artery occlusion), radiation-induced carotid stenosis, medical comorbidity, high carotid bifurcation, previous carotid endarteretomy 등에서 PTAS를 시행하고 있는바, 이 시술에 따르는 합병증은 다음과 같다.
  
Intimal dissection
  
Stent thrombosis:정맥 혹은 동맥내 antiplatelet 투여로 치료 할수 있다.
  
Distal embolization:low-profile balloon 혹은 filter가 색전형성의 예방에 도움이 된다.
  
Groin hematoma
  
Bradycardia and hypotension:cranial Ⅸ 신경의 자극으로 인하여 말초성 혈관 확장과 서맥을 유발할 수 있다.
  
Restenosis:혈관내막증식(intimal hyperplasia) 혹은 죽종판(atheromatous plague) 형성으로 유발된다.
  
Death:CAVATAS(Carotid and Vertebral Artery Transluminal Angioplasty Study)11)에 의하면 251명 중 6.4%에서 disabling stroke나 사망자가 발생하였다.

요     약

   여러 가지 뇌 혹은 소뇌질환에서 발생할 수 있는 뇌허혈 혹은 뇌경색을 예방하거나 뇌를 보호하기 위하여 활용되고 있는 revascularization procedure에는 STA-MCA anastomosis나 saphenous vein 혹은 radial artery를 이용하는 bypass graft와 같은 SR과 thrombolysis나 PTAS를 통한 ER로 나눌 수 있다. High-Flow bypass를 위한 SR의 중요한 합병증으로는 뇌내출혈, 혈전증, 색전증과 재협착 등을 들수 있다.
